PHOCUSED plays a unique role in our nonprofit community: providing an independent perspective from direct service providers while maintaining the close community connections that enable us to bring organizations together to see change enacted. By partnering with these organizations, PHOCUSED is able to serve those in our community that are especially vulnerable and contribute a robust health and human services sector.
From 2015-2017 PHOCUSED participated with our Continuum of Care for Oahu, through a grant from Aloha United Way, by facilitating Case Conferencing for single adults experiencing homelessness. Additionally, through a generous opportunity provided by the Hawaii Community Foundation’s HousingASAP Program, PHOCUSED served as the Coordinated Entry System (CES) Administrator for Families experiencing homelessness. During this one-year contract, PHOCUSED executed the HUD-mandated process, which allows resources to be more effectively directed to those families most in need.
As the Administrator for Families CES, PHOCUSED worked with Family Service and Housing Providers to perform matching and referrals for families. Families CES was conducted in collaboration with Oahu’s Continuum of Care (Partners in Care) and the Balance of State Continuum of Care (Bridging the Gap).
Finally, PHOCUSED serves in a liaison capacity for the Department of Human Services’ ‘Ohana Nui Initiative and the nonprofit community.
